24-Hour Customized Living is Minnesota's Medicaid Home and Community-Based Services (HCBS) waiver standard for residential assisted living with continuous staffing. At Horizon, it means a trained, awake staff member is present in the home around the clock — every night, every weekend, every holiday.

Unlike "on-call" arrangements where a caregiver might respond by phone, our model keeps staff on-site. If you need help at 3 a.m., someone is already there.

Care is delivered through an individualized service plan developed with the resident, their family, and their care coordinator. The plan is reviewed regularly and adjusted as needs change.

Who qualifies

Adults who need more daily support than an independent living arrangement can provide, but who can be safely and comfortably served in a small residential home rather than a nursing facility. Eligibility is typically established through a county needs assessment.

Getting started

Contact your county case manager or care coordinator to request a customized living assessment. Horizon can work directly with your lead agency to set up a service agreement once eligibility is confirmed. Contact us and we'll guide you through the process.

Each resident's service plan is assembled from the components below based on their assessed needs. Nothing is one-size-fits-all.

Personal Care & Daily Living

Hands-on assistance with bathing, dressing, grooming, oral hygiene, and personal routines. Delivered with patience, respect, and privacy — always at the resident's pace.

Medication Management

Staff assist with medication setup, administration reminders, and documentation. We coordinate with prescribing providers and pharmacies and flag any concerns to the care team.

Mobility & Transfer Assistance

Help with safe positioning, transfers (bed to chair, chair to standing), walking support, and the use of mobility aids — with falls prevention as a constant priority.

Meals & Nutrition

Three nutritious meals prepared daily in-home, with accommodations for dietary restrictions, preferences, and texture needs. Snacks available throughout the day.

Household Management

Housekeeping, laundry, linen changes, and general upkeep of the resident's living space and common areas — keeping the home clean, safe, and comfortable.

Socialization & Engagement

Planned and informal activities — music, reading, crafts, outings, conversation — that keep residents connected to the world around them and to each other.

Health Monitoring & Delegated Nursing

Regular health observations, vital sign monitoring, and delegated nursing tasks (such as wound care or catheter management) performed under a registered nurse's supervision.

Appointment & Transportation Support

Staff assist residents with scheduling and attending medical and social service appointments, coordinating transportation, and communicating with providers on the resident's behalf.

Reach out

Call or email us. We'll have a simple conversation about your situation — no forms, no intake packets, no pressure. Just a chance to get to know each other.

Step 02

Tour the home

Come see Horizon in person. Walk through the rooms, meet the staff, and ask every question you have. We want you to feel what the home is like before you decide anything.

Step 03

Care assessment

Your county case manager or care coordinator conducts a needs assessment. If you already have one, we can start from there. Horizon staff participate to ensure the plan fits the home.

Step 04

Move in

Once a service agreement is signed, we work with you on a move-in timeline that feels right. Your space, your things, your rhythm — we adjust to you.

Care shouldn't depend on
what you can pay.

Horizon accepts Minnesota Medicaid HCBS waivers for 24-Hour Customized Living. Waivers are issued by the state but managed through your county, which assigns a case manager to coordinate your care plan and funding.

CADI Waiver
Community Access for Disability Inclusion — for adults with physical disabilities, including those under age 65, who would otherwise require nursing facility level care.
Elderly Waiver (EW)
For adults 65 and older who need nursing facility-level care but prefer to receive services in a community residential setting like Horizon.
Brain Injury Waiver (BI)
For adults who have experienced a brain injury and require residential support and assistance to live safely outside of a hospital or nursing facility.
Private Pay
Horizon also welcomes residents paying privately or through long-term care insurance. Contact us to discuss rates and availability.
